New USAging Report Explores Coordinating Health and Community Care in Rural and Tribal Communities

USAging is pleased to release a new report titled “Voices From the Field: Insights on Coordinating Health and Community Care in Rural, Frontier, and Tribal Communities.” Presented by USAging’s Aging and Disability Business Institute and the Center of Excellence to Align Health and Social Care, the report is based on a series of key informant interviews with local leaders from Area Agencies on Aging (AAA) and Tribal organizations across the nation. This report features national trends in contracting for AAAs, community-based organizations (CBOs), community care hubs (CCHs), Tribal organizations and health partners that serve rural communities. The report highlights key themes, opportunities and challenges identified through conversations with 44 experts representing 24 rural and Tribal organizations across 20 states.

 

In addition to chronicling national trends in the coordination of health and community care in rural communities, the report includes specific best practices in the following areas:

  • Deploying preventive and chronic condition management services and supports that address upstream drivers of health.
  • Identifying cost savings for CBOs and health partners through health care partnerships.
  • Diversifying funding streams to increase sustainability and decrease reliance on limited, traditional funding sources.
  • Increasing efficiencies, scaling services and improving quality of care through participation in a CCH.
