USAging’s Strengthening Social Connection in Communities (co-managed by the Aging and Disability Business Institute and Engaged: USAging’s Resource Center for Engaging Older Adults) released a report highlighting the impressive achievements of the United Disabilities Services Foundation (UDSF), winner of The John A. Hartford Foundation 2025 Business Innovation Award.
United Disabilities Services Foundation (UDSF) was honored for its partnership with WellSpan Ephrata Community Hospital and its innovative Advanced Care Management Solutions (ACMS) program. UDSF teamed up with WellSpan Ephrata Community Hospital to embed CBO care managers directly within hospital discharge teams, addressing complex community needs through immediate, early intervention, flexible resources, and a rigorous six-month post-discharge follow-up model.
