Health Care-CBO Partnerships: Improving Health Outcomes & Social Connection

The 2026 winners of The John A. Hartford Foundation Business Innovation Award will highlight innovative partnerships and contracting models that promote social connection and employ creative strategies to achieve meaningful health outcomes. The Agency on Aging of South Central CT collaborates with Yale School of Medicine to provide sustainable, age-friendly healthcare through a Service Navigator. Nevada Senior Services partners with University Medical Center to facilitate dementia-capable care transitions that emphasize social connection after hospitalization. Fairfax Area Agency on Aging maintains its 36-year partnership with Inova Health. These national award winners will share program strategies, present examples and case studies, and answer audience questions.
