Join this webinar presented by USAging’s Aging and Disability Business Institute and the Center of Excellence to Align Health and Social Care to dive deeper into “Voices From the Field: Insights on Coordinating Health and Community Care in Rural, Frontier, and Tribal Communities,” a recent report that features national trends in contracting for Area Agencies on Aging (AAA), community-based organizations (CBOs), community care hubs (CCHs), Tribal organizations and health partners that serve rural communities. Presenters will be sharing data from a rural contracting environmental scan, discussing how the report relates to Rural Health Transformation opportunities and sharing practical approaches to health care contracting and network development in rural settings. During the webinar, USAging will also share forthcoming resources related to addressing the needs identified in the rural health care contracting environmental scan over the next year. This session is intended for AAAs, Title VI Programs, CCHs and other CBOs that serve rural areas and are exploring or participating in contracts with health care entities.
