SUMMARY:Cultivating Community Care Hubs: An Evolving Model to Improve Alignment Between Health and Social Care Services
DESCRIPTION:Among health systems\, healthcare providers\, and health insurers\, the importance of addressing SDOH — particularly for those with complex care needs — has come into sharp focus.  \nThe Partnership to Align Social Care is a cross-sector effort to co-design a multi-faceted strategy to enable successful partnerships between healthcare organizations and networks of CBOs\, or Community Care Hubs.  \nJoin the Partnership and health and social care sector leaders for a deep dive into the important role of Community Care Hubs in promoting an equitable health and social care ecosystem.  \nAttendees will gain:  \n\nA brief overview of the Partnership to Align Social Care and the effort to achieve an aligned health and social care system that reflects and incorporates the vital voice of the community; \nBackground on the evolution of the Community Care Hub concept and its value proposition; \nPerspective from CBO and healthcare leaders detailing their experience in building and contracting with CBO networks and hubs.
