The John A. Hartford Foundation (JAHF) Business Innovation Award recognizes innovative and transformative social connection initiatives by community-based organizations working with health care entities, with the goal of replicating these successful partnerships nationwide. The 2025 winners, United Disabilities Services, Elder Services of Worcester Area, and Senior Connection, will discuss partnerships and contracting models for social connection, as well as creative ways to achieve meaningful outcomes for older adults and their caregivers. These national award recipients will share their strategies for developing contracts and partnerships, the challenges they faced and the outcomes they achieved.
