SUMMARY:How to Engage in Cultural Humility When Working with Minoritized Individuals Ages 65 and Older

SUMMARY:How Livable Communities are Addressing Social Determinants of Health
DESCRIPTION:The next installment of TFAH’s 2022 Age-Friendly Public Health Systems training series will focus on how livable communities are addressing social determinants of health. Speakers will share national and local level innovative practices in livable communities and note valuable partnerships with departments of public health. AARP defines a livable community as one “that is safe and secure. It offers choices in where to live and how to get around. And it equitably serves residents of all ages\, ability levels\, incomes\, races\, ethnicities\, and other backgrounds.” Having safe\, healthy\, and livable communities where older adults can remain active and engaged is an important part of healthy aging.

SUMMARY:Pre-Conference Intensive: Improving Housing Stability Through Health Care Contracts
DESCRIPTION:REGISTER FOR THIS EVENT HERE \nThe Aging and Disability Business Institute is hosting a pre-conference intensive from 8:30 AM – 12:00 PM CT that will discuss how to improve housing stability through health care contracts.  \nHousing instability is associated with poor health\, increased risk for falls\, and avoidable emergency department visits and hospitalizations. Keys to improving a person’s ability to continue to live in their home—also known as housing stability—include affordable\, accessible\, and safe housing with ready access to home and community-based services. AAAs can use their OAA funding to support housing stability\, but health care contracting can enable them to serve a fuller range of individuals’ needs and/or expand the number of older adults and caregivers they serve. Learn how health care contracts can support critical housing stability functions. Hear from federal presenters on some opportunities available through the American Rescue Plan Act and learn about promising AAA partnerships with housing and health care providers. \nPresenters: \n\nBecky Kurtz\, Director\, The Atlanta Regional Commission (ARC) \, Atlanta\, GA \nJennifer Raymond\, Chief Strategy Officer\, AgeSpan\, Lawrence\, MA \nKelly Cronin\, Deputy Administrator\, Center for Innovation and Partnership\, Administration for Community Living \, Washington\, DC \nPatty Bordie\, Director \, Capital Area Council of Governments\, Austin\, TX \nMaya Op de Beke\, Program Manager\, USAging\, Washington\, DC \nTraci Murray\, Region 6 Assitant Regional Administrator\, Substance Abuse and Mental Health Services Administration\, Washington\, DC \n\nThe USAging Answers on Aging Annual Conference and Tradeshow is the country’s largest gathering of local leaders in aging. SUMMARY:Integrating Health and Social Services through a Novel Independent Practice Association
DESCRIPTION:REGISTER FOR THIS WEBINAR HERE. \nThis webinar introduces a new S4A study investigating the impact of a novel independent practice association (IPA) formed among community-based social service organizations (CBOs) to address social determinants of health among residents of upstate New York. The Healthy Alliance IPA allows diverse CBOs offering services for housing\, transportation\, food\, and other social needs to join together in a shared-governance association that facilitates referrals\, care coordination\, and performance-based contracting with health plans and medical providers. A quasi-experimental research design is used to compare patients residing in counties served by the Healthy Alliance IPA with a matched comparison group of patients from other counties\, using measures of healthcare utilization\, social service utilization\, and total cost of care. The study focuses specifically on the outcomes experienced by racial and ethnic minority populations and also examines the roles played by CBOs led by Black\, Indigenous\, Latino and other persons of color. The research team is led by Stanford University School of Medicine in partnership with the Healthy Alliance IPA\, Albany County Department of Health\, Rensselaer County Department of Health\, and other community partners.
