DESCRIPTION:USAging is celebrating 50 years! The first Answers on Aging Conference was a gathering of a few hundred Aging Network professionals held in 1975 at the Mayflower Hotel in Washington\, DC.  \n\nAs we head to Chicago in 2025\, the USAging Answers on Aging Annual Conference and Tradeshow has become the country’s largest gathering of local leaders in aging well at home\, attracting more than 1\,400 Area Agencies on Aging; tribal aging programs and other federal\, state and local leaders in aging; home and community-based services; and caregiving from across the nation.  \n\nThe USAging Conference has a rich history of providing tremendous opportunities to spur innovation; learn about best and promising practices; network and learn from colleagues; and gain insight into federal policy and national trends that affect older adults\, people with disabilities and their caregivers. This is a must-attend conference for anyone working in the Aging Network or wanting to learn more about the services and supports that help older adults thrive in their communities. Hartford Foundation\, will highlight the strengths\, challenges and replicable aspects of this innovative partnership. The webinar will feature a panel discussion with leaders from case study participating organizations\, sharing their real-world experiences\, key takeaways and promising practices for other CILs and AAAs looking to build/replicate similar connections and collaborations between entities.  \nThe CIL-AAA partnership featured in this case study reflects how the two entities operate successfully\, demonstrating meaningful collaboration between disability and aging services are possible. Service delivery and resource accessibility are enhanced by bridging services gaps ultimately improving access for older adults and people with disabilities. As a result of this CIL-AAA innovative partnership\, the organizations have unlocked funding opportunities from within the health care sector to facilitate the provision of crucial services such as transition and diversion for aging and disabled individuals.

DESCRIPTION:Join the Aging and Disability Business Institute in partnership with ILRU for a discussion with colleagues from the U.S. Administration on Community Living (ACL) and Centers for Independent Living (CIL) on the value of the community care hub (CCH) model in supporting the advancement of independent living and bringing social care expertise to health care organizations. \nWith increased attention on the impact of health-related social needs (HRSNs)\, CILs are positioning themselves to partner and contract with health care organizations in providing social care services that are core to independent living. A growing number of CILs and other community-based organizations (CBOs) are entering into contracts as part of a CBO network led by a CCH. CCHs provide administrative and operational infrastructure for a network of CBOs and enable efficient contracting and sustainable financing to better serve populations in need. \nThis webinar will define and introduce the CCH model\, illustrate the various roles for CILs in engaging in a CCH-led network\, and feature CIL leaders and their experiences in partnering with a CCH and operationalizing health care contracts through the CCH model.

SUMMARY:Community Care Hub 101 Learning Series: Building the Community Care Hub Business Case
DESCRIPTION:The Administration for Community Living (ACL)\, in collaboration with the Center of Excellence to Align Health and Social Care (powered by USAging’s Aging and Disability Business Institute)\, invites community-based organizations serving as community care hubs (CCHs) or interested in learning more about the community care hub model to participate in an upcoming four-part Community Care Hub 101 Learning Series. The series will take place on four Thursdays in mid-November through December 2023. \nA community care hub (CCH) is a community-focused entity that organizes and supports a network of community-based organizations providing services to address health-related social needs. It centralizes administrative functions and operational infrastructure\, including but not limited to\, contracting with health care organizations\, payment operations\, management of referrals\, service delivery fidelity and compliance\, technology\, information security\, data collection\, and reporting. \nEach webinar will focus on the foundational elements of serving as a community care hub. The webinars will showcase examples from leading community care hubs as well as resources available to support community-based organizations in forming\, operationalizing\, and launching a hub in their community. \nRegister today for the Community Care Hub 101 Learning Series. SUMMARY:Operationalizing Contracts: Improving Contracting Implementation and Collaboration
DESCRIPTION:Contracting to Align Health and Social Care Ecosystems: A Webinar Series Sharing Leading Practices \nJoin the Partnership to Align Social Care\, the Aging and Disability Business Institute\, and Camden Coalition for a webinar series offering a deep dive into leading contracting practices to effectively and efficiently address HRSNs. \nEach webinar in the series will focus on the essential elements of contracting between health care and community partners and feature leading practices from the field. Each event will also highlight key findings from a four-part toolkit\, Healthcare Guide to Contracting with CBOs\, developed with extensive input from CBOs\, health plans\, and health systems. While these resources were developed for a healthcare audience\, CBOs\, CCHs\, and other health and social care ecosystem stakeholders will benefit from understanding how healthcare organizations approach the contracting process.
