DESCRIPTION:Join USAging’s Aging and Disability Business Institute for an insightful webinar where speakers will dive into a compelling peer-led case study showcasing the unique\, powerful partnership between a Center for Independent Living (CIL) and an Area Agency on Aging (AAA) operating collaboratively as an Aging and Disability Resource Center (ADRC).  \nThis case study\, developed by Disability Revolution\, Inc. for the Aging and Disability Business Institute in partnership with ILRU\, and funded by The John A. Hartford Foundation\, will highlight the strengths\, challenges and replicable aspects of this innovative partnership. The webinar will feature a panel discussion with leaders from case study participating organizations\, sharing their real-world experiences\, key takeaways and promising practices for other CILs and AAAs looking to build/replicate similar connections and collaborations between entities.  \nThe CIL-AAA partnership featured in this case study reflects how the two entities operate successfully\, demonstrating meaningful collaboration between disability and aging services are possible. Service delivery and resource accessibility are enhanced by bridging services gaps ultimately improving access for older adults and people with disabilities. As a result of this CIL-AAA innovative partnership\, the organizations have unlocked funding opportunities from within the health care sector to facilitate the provision of crucial services such as transition and diversion for aging and disabled individuals.

DESCRIPTION:Join the Aging and Disability Business Institute in partnership with ILRU for a discussion with colleagues from the U.S. Administration on Community Living (ACL) and Centers for Independent Living (CIL) on the value of the community care hub (CCH) model in supporting the advancement of independent living and bringing social care expertise to health care organizations. \nWith increased attention on the impact of health-related social needs (HRSNs)\, CILs are positioning themselves to partner and contract with health care organizations in providing social care services that are core to independent living. A growing number of CILs and other community-based organizations (CBOs) are entering into contracts as part of a CBO network led by a CCH. CCHs provide administrative and operational infrastructure for a network of CBOs and enable efficient contracting and sustainable financing to better serve populations in need. \nThis webinar will define and introduce the CCH model\, illustrate the various roles for CILs in engaging in a CCH-led network\, and feature CIL leaders and their experiences in partnering with a CCH and operationalizing health care contracts through the CCH model.
