This data brief provides an overview of data on community care hubs (CCHs) and networks gathered through several sources: USAging’s 2023 CBO–Health Care Contracting Survey, data from the Center of Excellence to Align Health and Social Care (COE) 2025 CCH Map Survey and the COE’s grantee reporting. It includes data on structure, geographic reach, network membership, health care partners, services provided through contracts and more.
